Welcome to Onyx Healthcare, Inc.’s d/b/a Onyx M.D. (“Onyx”) website (“Site”).  Onyx understands that the privacy of your personal information is important to you and is committed to protecting your online privacy.  Our privacy policy ("Privacy Policy") set forth here describes how we may use and disclose personally identifiable information ("Personal Information") that we may collect about you through this Site.  This Privacy Policy is subject to change.  We recommend that you check this page regularly to familiarize yourself with any changes.

Information Collected and Use of Information:

We are the sole owners of the information collected on this Site.  We have access to and collect information that you voluntarily give us via email or other direct contact from you.  While we may collect information from you and use this information to respond to you regarding the reason you contacted us, we will not disclose your social security number, name, telephone number, credit card or other Personal Information you provide to us to third parties outside of our organization unless you authorize us to do so, except where needed to deliver a service or product ordered or requested by you, or in the limited circumstances mentioned below.  Additionally, we will give you the ability to access and correct your Personal Information at any time.

Onyx uses the information that you provide to establish a staffing placement relationship with you, to provide you with job placement services, and for other business related purposes.  Some of these business related purposes may include, fulfilling your requests for information, arranging for employment opportunities for you, providing payroll services, assessing whether or not you are qualified for a position, providing referral services, performing our service agreements, enforcing payment arrangements, and for correspondence, risk management, and administrative purposes, such as performing research and analyses and improving our Site and information systems.

Information that You Provide to Us:

Some areas of our Site may require that you provide Personal Information in order to access all of the Site’s information and features.  The Personal Information that you provide will only be used for the purposes specified at the time of provision, including contacting you regarding employment opportunities.  You also may elect to upload a resume or curriculum vitae, which will provide any Personal Information you have chosen to disclose to us by including it in the file you upload.  If you decide to contact us directly, we will receive the contact information you provide so that we are able to respond, as well as any other Personal Information that you chose to include in your message.

 

You may elect not to provide us with all of the information we request. However, if you do not provide the information we have requested, we may not be able to provide you with an account or to use your communication to facilitate job placement.

Information that We Derive Independently From Your Use of the Site:
